  • Open Enrollment application is for student(s) that want to attend a school outside of their zoned school of enrollment. Open Enrollment applications are approved or denied based on the number of students already enrolled in the grade level and school you wish your student to attend. 

    A new student enrollment form is also required to complete the enrollment process.

  • Some educational programs are only offered in a limited number of school such as specific special education programs and English Language Learner programs. Contact the Director of Federal Programs for further information.

Submitting this form with count as your digital signature. Misrepresentation of information on this application may result in revocation of the applicant's approval to attend a Jefferson School District school.
